PLEASE SUPPORT excellent programs in various social service and healthcare provider organizations focused on helping people improve their outcomes in health, housing, jobs, education, and other life arenas. As part of these programs, the family participants (clients/patients) and staff will be empowered by using the OurREACH™ software platform and app.

BACKGROUND
Case managers, community health workers, and/or others working with families and individuals are seeking ways to further engage and empower these families and individuals to improve outcomes. These organizations are also seeking ways to support their staff, who are stretched thin.
To do so , they will launch programs in which their staff and the people they serve will use the software platform/app called OurREACH™ to support and extend these trusted relationships and improve client/patient outcomes in health and Social Drivers of Health such as housing, jobs, education, financial literacy, etc. OurREACH™ also supports parenting programs. OurREACH™, developed by Lambent Data, is a HIPAA-compliant, collaborative software platform and app, with data analytics.
TYPES OF ORGANIZATIONS
These organizations span those helping:
*FAMILIES with YOUNG CHILDREN -- Head Start programs, Early Head Start programs, maternal/infant services, etc.
*OLDER YOUTH -- Organizations helping foster youth "aging out," workforce/career development programs for youth who left high school without graduating, etc.
*MANY AGES -- Community health clinics serving individuals and families, comprehensive family services, etc.
Current organizations using OurREACH™ are based in New Jersey and Florida. OurREACH™ can be used by organizations across the U.S. and in Puerto Rico.
Lambent Data staff have been talking with leaders of additional organizations who are excited to use OurREACH™ in these programs. YOUR DONATIONS will help the organizations to "jumpstart" these programs, to start quickly outside of their typical budget cycle.

This occurs through:
* Action-oriented, personalized Milestones, Goals, integrated resources, and communication re. health & Social Drivers of Health
* Fits “between visits” in Continuum of Care for family empowerment toward Milestones
* AI elements extend staff capacity, to use in “light touch” or more intensive ways
* Customizable platform integrates with workflows & Electronic Health Record systems / Constituent Relationship Management systems (incl. via Redox)
JUMPSTARTER VALUE OF YOUR DONATIONS
Your donations will provide an innovative "jumpstarter" opportunity for these organizations to start quickly with OurREACH™ -- to begin increasing user engagement, improving outcomes, reporting in more detail, etc.
Longer term funding will occur in several ways:
* Organizations build OurREACH™ into their budget cycle.
* Some of these programs qualify for Medicaid Waiver Initiative funding in the future.
* Other insurance companies partner with the organizations for these programs in health and Social Drivers of Health.
* OurREACH™ leads to improved outcomes and more detailed reporting on HOW clients/patients are progressing -- which leads to more funding from the organizations' individual and foundation donors for growth of the programs.
